Red blood cell (RBC) lifespan represents the earliest and most direct indicator of erythrocyte destruction. Under physiological conditions, the average lifespan of human red blood cells is approximately 120 days. In August 2019, the Chinese Medical Association released a clinical consensus recommending the exhaled carbon monoxide (CO) test as the preferred approach for assessing RBC lifespan in hematological disorders. According to this consensus, hemolysis as an etiological factor of anemia can be excluded if the RBC lifespan exceeds 70 days. An RBC lifespan ranging from 50 to 70 days is suggestive of hemolysis, whereas a lifespan shorter than 50 days confirms the presence of hemolysis. Furthermore, the severity of hemolysis correlates inversely with RBC lifespan. RBC lifespan is determined by quantifying exhaled carbon monoxide, a metabolic byproduct generated during erythrocyte breakdown. This measurement enables the calculation of the erythrocyte destruction rate, thereby allowing estimation of the patient’s RBC lifespan. The detection device employed in this method is developed by Shenzhen Xianya Biotechnology Co., Ltd., and has been certified as a National Class II Innovative Medical Device. Compared with the ⁵¹Cr labeling method utilizing well-type gamma counters, this technique improves detection efficiency by nearly 3,000‑fold.
